We tested 16 South Florida dental practices. 13 were completely invisible to AI — including multi-location practices spending thousands monthly on Google Ads.
Dental advertising is competitive — not as expensive per click as emergency services, but the math on patient value changes the equation entirely. A single dental implant patient is worth $5,000 to $12,000 over the course of treatment. Even a general dentistry patient averages $800–$1,500 per year in recurring revenue. That means every click you pay for has real stakes.
But a growing number of prospective patients aren't clicking ads at all. They're asking ChatGPT.
"Best dentist in Boca Raton for dental implants" — typed into ChatGPT right now by someone ready to book a consultation. Which practice got recommended? Almost certainly not the one with the biggest ad budget.
We ran this test across 16 dental practices in Palm Beach and Broward counties. Here's what the data showed.
These are current average costs per click for dental-related searches in South Florida:
| Search Term | Avg. Monthly Volume | Cost Per Click |
|---|---|---|
| dentist near me | 2,085,000 | $13.90 |
| dental office near me | 165,000 | $13.21 |
| orthodontist near me | 246,000 | $20.38 |
| dental implants near me | 110,000 | $29.86 |
| teeth implants near me | 90,500 | $30.33 |
The volume on "dentist near me" — over 2 million searches a month — is among the highest of any local service category. When even a fraction of that traffic shifts to AI-first searches, the practices that aren't visible lose real scheduled appointments.
We asked ChatGPT, Gemini, and Google's AI Overview to recommend dentists and orthodontists across Palm Beach and Broward counties. We tested each of the 16 practices against their primary city and service type.
The pattern was consistent with our broader research across contractors, roofers, and plumbers: AI recommendations track organic signals, not paid advertising. A large multi-location dental group spending $10,000 a month on ads showed up exactly the same as a solo practice spending nothing — invisible.
Not all dental searches are equal. A patient searching "teeth cleaning near me" might be worth $200. A patient searching "dental implants near me" or "orthodontist near me" represents a $5,000–$15,000 treatment decision — and they're doing more research before booking.
That research increasingly includes asking AI. And for these high-value searches, AI tools are recommending whoever has the strongest organic trust signals — detailed service content, consistent citations, and recent location-specific reviews. A practice that has invested heavily in paid search but neglected organic SEO gets nothing.
AI tools heavily weight whether your website ranks organically for the specific procedure and city. If you're not on page 1 for "dental implants [your city]" or "orthodontist [your city]," you almost certainly aren't appearing in AI recommendations for those searches either. The correlation across our testing was close to 1:1.
Your name, address, and phone number need to be consistent across Google, Yelp, Healthgrades, Zocdoc, and dental-specific directories. For dental practices, even minor inconsistencies — a suite number formatted differently, an old phone number on one directory — create distrust signals that push you out of AI results.
Reviews that name specific procedures and locations carry significantly more weight than generic five-star ratings. "Amazing implant experience at this Delray Beach office" outperforms 200 generic reviews from two years ago. AI is looking for evidence that you do what you say you do — in the location where the patient is searching.
Every day, prospective patients in your area ask AI which dentist to choose for their specific need. AI gives them a short list. If your practice isn't on it, one of your competitors is — and they get the consultation request without spending a dollar on ads.
For high-value procedures, that's not a minor inconvenience. A dental implant patient that should have found you but found a competitor instead represents $5,000–$12,000 in treatment revenue that walked out the door before you ever knew they existed.
Based on our testing, the odds are roughly 4 in 5 that your practice is one of the invisible ones. The question is whether you know it — and what your specific gaps are.
